Protected Health Information

Entra Health Systems is fully compliant with HIPAA requirements and standards in the US as required by law to maintain the privacy of patient protected health information and to provide patients with notice of its legal duties and privacy practices regarding protected health information.

It is the policy of Entra Health Systems that all employees preserve the integrity and the confidentiality of Protected Health Information (PHI) pertaining to the patients we serve. The purpose of this policy is to ensure that we have the necessary medical information and PHI to ensure payment for delivered services while protecting the confidentiality of the PHI of the patients we serve to the highest degree possible.

At the core of the MyGlucoHealth Patient Analytics Portal is a relational database architecture designed to track, audit, route, and store patient uploads, analysis and personal healthcare data. Identifying information relating to a specific patient’s records is stored in an isolated database repository.

By encrypting the patient’s identity, as well as separating the physical storage of the data, we further ensure that no unauthorized party can interpret, decipher, or decode patient records maintained within the database, even if the data were accessed directly from an authorized domain account on the server itself.

Data Storage

As a standard policy, no patient data is managed or stored at Entra Health Systems. All production client data is managed at our secure off site hosting provider. The identity of our hosting provider is privileged and confidential information.

Confidentiality

Entra Health Systems requires each employee and independent contractor to enter into a written agreement that states all Confidential Information, and all embodiments received or developed by them while employed by Entra Health Systems, be held in trust and in the strictest confidence.

Hosting Provider

Entra Health Systems’ secure off site hosting provider has very stringent policies and controls to insure that patient data is never jeopardized. The Hosting Center does not have any level of access to any Entra Health Systems networks or systems other than those physically located at the Hosting Center.
