The MyGlucoHealth Network

The MyGlucoHealth Network is an integrated disease management technology that provides comprehensive tools for diabetic patients, physicians, clinical staff and caregivers. Central to the MyGlucoHealth Network is the Patient Analytics Portal at www.myglucohealth.net. The Portal is an extensive communication and data management web site that provides an easy to use interface for automatically collecting test results from every MyGlucoHealth patient.

Together with the MyGlucoHealth meter, the Portal offers a complete solution for managing the communications flow needed for patients and their physicians to effectively treat their diabetes. Test results are automatically sent to the patient’s private, secure page where they can track and chart test results and other related data. In addition patients can send related health information to their physician and schedule appointments online using the Portal.

The Portal is designed to help the patient take “ownership” of their disease management so they can make better lifestyle choices. Test results stored on the meter are wirelessly transmitted by PC or mobile phone to the MyGlucoHealth Portal for posting. This is performed automatically using Bluetooth technology or by connecting a USB cable directly to the meter.

On the MyGlucoHealth Portal readings are interpreted and charted in a number of different ways to best exhibit patient data. This process completely eliminates the manual logging of test results, improving data accuracy and removing the opportunity for inaccurate or “manufactured” entries.

In addition to patient postings of blood glucose readings, a public area of the MyGlucoHealth Portal features an expanding collection of in-depth diabetes treatment and lifestyle resources. Data on the Portal is accurate, secure, gathered in real-time and accessible to both patients and clinicians 24 hours a day, seven days a week from any Internet-connected computer on the globe.
